Circular: Supply Constraints with Rifampicin-based combinations for TB treatment - interim therapeutic alternatives

TB Treatment Alert: National RH Supply Shortage—Rifapentine & Isoniazid Recommended as Alternative to Prevent Disruptions!

Guidance on Alternative Treatment Options Amid RH Supply Constraints

The Primary Healthcare, Adult Hospital-Level, and Pediatric Hospital-Level Standard Treatment Guidelines (STGs) and the Essential Medicines List (EML) currently endorse the use of rifampicin/isoniazid (RH) 150/75 mg and 300/150 mg for the continuation phase of tuberculosis (TB) treatment and TB preventive therapy.

However, due to an insufficient national supply of RH, current stock levels are inadequate to meet clinical demand. To ensure uninterrupted patient care, the use of rifapentine in combination with isoniazid may be considered as an alternative for the continuation phase of treatment.

While acknowledging the limited clinical data supporting this substitution, this approach represents a pragmatic response to mitigate treatment disruptions resulting from medication shortages.

Healthcare professionals are encouraged to consult the official circular for further details and clinical guidance.
