Influences of Western Cape community service doctors’ choice regarding public, rural practice

Despite South Africa's compulsory community service aimed at boosting rural healthcare, few doctors intend to stay in these areas (8%). A Western Cape study found rural upbringing (6.5x more likely) and rural placements (7.7x/3.5x more likely) significantly influenced retention. Policies should prioritise rural background students and placements, alongside ensuring personal safety, job satisfaction, and mental health in rural facilities.

Background: Staff shortages in rural areas have resulted in unequal access to healthcare in South Africa. The compulsory community service programme aims to address this disparity, but to be effective, it must encourage doctors to remain in rural facilities beyond their service periods. Identifying factors that influence their decisions to stay is crucial for developing strategies to improve rural doctor retention.
