Prescribing points

Method of administration

  • Ixekizumab is for subcutaneous administration. Injection sites may be alternated. If possible, areas of the skin that show psoriasis should be avoided as injection sites
  • The solution must not be shaken
  • After training in subcutaneous injection technique, a patient may self-inject ixekizumab. However, the physician should ensure appropriate follow-up of patients
  • Doses less than 80mg which require dose preparation should only be administered by a healthcare professional
  • In order to improve the traceability of biological medicinal products, the name and the batch number of the administered product should be clearly recorded

Traceability

  • In order to improve the traceability of biological medicinal products, the name and the batch number of the administered product should be clearly recorded

Infections

  • Ixekizumab should be used with caution in patients with clinically important chronic or active infection, including HIV-positive patients. If such an infection develops, monitor carefully and discontinue ixekizumab if the patient is not responding to standard therapy or the infection becomes serious. Do not resume ixekizumab until the infection resolves
  • Patients should be instructed to seek medical advice if signs or symptoms suggestive of an infection occur
  • If an infection develops, patients should be carefully monitored and ixekizumab discontinued if the patient is not responding to standard therapy or if the infection becomes serious
  • Ixekizumab should not be resumed until the infection resolves
  • Ixekizumab must not be given to patients with active tuberculosis (TB). Anti-TB therapy prior to initiation of ixekizumab in patients with latent TB should be considered

Inflammatory bowel disease (including Crohn's disease and ulcerative colitis)

  • Cases of new or exacerbations of inflammatory bowel disease have been reported with ixekizumab
  • Ixekizumab is not recommended in patients with inflammatory bowel disease
  • If a patient develops signs and symptoms of inflammatory bowel disease or experiences an exacerbation of pre-existing inflammatory bowel disease, ixekizumab should be discontinued and appropriate medical management should be initiated

Immunisations

  • Ixekizumab should not be used with live vaccines
  • No data are available on the response to live vaccines; there are insufficient data on response to inactive vaccines

Mechanism of action

Ixekizumab is an IgG4 monoclonal antibody that binds with high affinity (<3pM) and specificity to interleukin 17A (both IL-17A and IL-17A/F). Elevated concentrations of IL-17A have been implicated in the pathogenesis of psoriasis by promoting keratinocyte proliferation and activation, as well as in the pathogenesis of psoriatic arthritis and axial spondyloarthritis by driving inflammation leading to erosive bone damage and pathological new bone formation. Neutralisation of IL-17A by ixekizumab inhibits these actions.
