Special populations

Pregnancy

  • Caution; positive evidence of risk
  • The safe use of pentazocine in pregnancy, other than labour, has not been established
  • Pentazocine is a narcotic analgesic that crosses the placenta, and prolonged use during pregnancy may result in neonatal opioid withdrawal syndrome
  • Use of opioids during pregnancy may be associated with birth defects such as poor foetal growth, preterm delivery, congenital heart defects and neural tube defects

Breastfeeding

  • Caution; not recommended
  • It is currently unknown if pentazocine becomes present in breast milk
  • A decision must be made whether to discontinue breastfeeding or to discontinue pentazocine therapy taking into account the benefit of breastfeeding for the child and the benefit of therapy for the mother

Fertility

  • Sexual dysfunction or infertility may occur in males and females following prolonged use of opioids due to secondary hypogonadism

Hepatic impairment

Incompatibilities

  • Pentazocine is incompatible in solution with diazepam or chlordiazepoxide and with alkaline substances such as aminophylline and soluble barbiturates
  • Such solutions should not be mixed together

Mechanism of action

Pentazocine is a potent analgesic. Pentazocine is an agonist of kappa opiate receptors and a partial agonist of mu opiate receptors in the central nervous system, resulting in inhibition of ascending pain pathways, modifying the perception and response to pain. It produces analgesia, respiratory depression and sedation similar to morphine-like opioids.
