Pregnancy & lactation
Pregnancy
South African recommendations
- Contraindicated
- Trimethoprim and sulfamethoxazole cross the placenta and their safety in pregnant women has not been established and should not be used during pregnancy
Recommendations in other jurisdictions
- Caution; not recommended
- Trimethoprim and sulfamethoxazole cross the placenta and their safety in pregnant women has not been established. Case-control studies have shown that there may be an association between exposure to folate antagonists and birth defects in humans
- Trimethoprim is a folate antagonist and, in animal studies, both agents have been shown to cause foetal abnormalities
- Trimethoprim/ sulfamethoxazole should not be used in pregnancy, particularly in the first trimester, unless clearly necessary. Folate supplementation should be considered if trimethoprim/ sulfamethoxazole is used in pregnancy
- Sulfamethoxazole competes with bilirubin for binding to plasma albumin. As significantly maternally derived drug levels persist for several days in the newborn, there may be a risk of precipitating or exacerbating neonatal hyperbilirubinaemia, with an associated theoretical risk of kernicterus, when trimethoprim/ sulfamethoxazole is administered to the mother near the time of delivery. This theoretical risk is particularly relevant in infants at increased risk of hyperbilirubinaemia, such as those who are preterm and those with glucose-6-phosphate dehydrogenase deficiency
Breastfeeding
South African recommendations
- Contraindicated
- The components of trimethoprim and sulfamethoxazole are excreted in breast milk
- Administration of trimethoprim and sulfamethoxazole should be avoided in lactating mothers where the mother or infant has, or is at particular risk of developing, hyperbilirubinemia and should not be given to the newborn infant during the first weeks of life
Recommendations in other jurisdictions
- Caution; not recommended
- Trimethoprim and sulfamethoxazole are excreted in breast milk. Administration of trimethoprim/ sulfamethoxazole should be avoided in breastfeeding mothers where the mother or infant has or is at particular risk of developing hyperbilirubinaemia
- Additionally, administration of trimethoprim/ sulfamethoxazole should be avoided in infants younger than eight weeks in view of the predisposition of young infants to hyperbilirubinaemia
Fertility
Recommendations below are applicable to South Africa and other jurisdictions
- No information available
Renal impairment
South African recommendations
Suspension and tablets
- CrCl >25ml/min: No dose adjustment recommended
- CrCl 15-25ml/min: Caution; dose adjustment recommended
Standard dosage for a maximum of 3 days followed by half the standard daily dosage - CrCl <15ml/min: Contraindicated
Not to be administered unless haemodialysis facilities are available when half the standard daily dosage may be given
Measurements of plasma concentrations of sulfamethoxazole at intervals of 2 days are recommended in samples obtained 12 hours after administration of trimethoprim/sulfamethoxazole
If the concentration of total sulfamethoxazole exceeds 150μg/ml then treatment should be interrupted until the value falls below 120μg/ml
No Information is available for children with renal failure
Injection
Adults and children >12 years
- CrCl >30ml/min: No dose adjustment recommended
- CrCl 15-30ml/min: Caution; dose adjustment recommended
- CrCl ≤15ml/min: Contraindicated
|
Creatinine Clearance (ml/min) |
Recommended Dosage |
| >30 | 100% of standard dose |
| 15-30 | 50% of standard dose |
| <15 | Not recommended |
Recommendations in other jurisdictions
Adults and children >12 years
- CrCl >30ml/min: No dose adjustment recommended
- CrCl ≤30ml/min: Caution; dose adjustment recommended
The elimination half-life of trimethoprim is increased by a factor of 1.5-3.0 when the creatinine clearance is less than 10ml/minute. When the creatinine clearance falls below 30ml/min the dosage of co-trimoxazole should be reduced - CrCl ≤30ml/min: Contraindicated (where repeated measurements of the plasma concentration cannot be performed)
|
Creatinine Clearance (ml/min) |
Recommended Dosage |
| >30 | Oral (tab):160mg TMP/800mg SMZ, 12hrly Oral (sus): 80mg TMP/400mg SMZ, 12hrly IV: 2 amp (10ml), every 12hrs |
| 15-30 | Oral (tab): 80mg TMP/400mg SMZ, 12hrly Oral (sus): 40mg TMP/200mg SMZ, 12hrly IV: 1 amp (5ml), every 12hrs |
| <15 | Not recommended |
Measurements of plasma concentration of sulfamethoxazole at intervals of 2 to 3 days are recommended in samples obtained 12 hours after administration of co-trimoxazole. If the concentration of total sulfamethoxazole exceeds 150 microgram/ml then treatment should be interrupted until the value falls below 120 microgram/ml
Children ≤12 years
- Caution; no information available
- No information available for children aged 12 years and under with renal failure
Hepatic impairment
South African recommendations
- Caution: no information available
Use is contraindicated in patients with liver parenchymal damage
Recommendations in other jurisdictions
- Mild to moderate: Caution; no information available
No data are available relating to dosage in patients with impaired hepatic function
Caution should be exercised when treating patients with severe hepatic parenchymal damage as there may be changes in the absorption and biotransformation of trimethoprim and sulfamethoxazole - Severe: Contraindicated
Elderly
South African recommendations
- Caution; no dose adjustment recommended
In elderly patients, a slight reduction in renal clearance of sulfamethoxazole but not trimethoprim has been observed (sus, tab)
Elderly patients with normal renal function should receive the usual adult dosage (inj)
Recommendations in other jurisdictions
- Caution; no dose adjustment recommended
Particular care is always advisable when treating elderly patients because, as a group, they are more susceptible to adverse reactions and more likely to suffer serious effects as a result particularly when complicating conditions exist, such as impaired renal and/or hepatic function and/or concomitant use of other drugs
In elderly patients, a slight reduction in renal clearance of sulfamethoxazole but not trimethoprim has been observed

Instructions for use (IV)
- Trimethoprim/sulfamethoxazole must be diluted before administration
- The following infusion solutions can be used for dilution:
- Glucose 5% and 10%
- Xylitol 10%
- Ringer's Solution
- Dextran 70.6% in glucose 5%
- Sodium chloride 0.9%
- Sodium chloride 0.45% + glucose 2.5% - Refer to product specific information for dilution schedules
Mechanism of action
Sulfamethoxazole competitively inhibits the utilisation of para-aminobenzoic acid in the synthesis of dihydrofolate by the bacterial cell resulting in bacteriostasis. Trimethoprim reversibly inhibits bacterial dihydrofolate reductase (DHFR), an enzyme active in the folate metabolic pathway converting dihydrofolate to tetrahydrofolate. Depending on the conditions the effect may be bactericidal. Thus trimethoprim and sulfamethoxazole block two consecutive steps in the biosynthesis of purines and therefore nucleic acids essential to many bacteria. This action produces marked potentiation of activity in vitro between the two agents.
