Maintenance dosing

  • SC: Given at week 8 after IV dose: 90mg, Then: 90mg, every 12 weeks

Patients who have not shown adequate response at 8 weeks after the first SC dose, may receive a second SC dose at this time. Patients who lose response on dosing every 12 weeks may benefit from an increase in dosing frequency to every 8 weeks

Patients may subsequently be dosed every 8 weeks or every 12 weeks according to clinical judgment. Consideration should be given to discontinuing treatment in patients who show no evidence of therapeutic benefit 16 weeks after the IV induction dose or 16 weeks after switching to the 8-weekly maintenance dose

Immunomodulators and/or corticosteroids may be continued during treatment with ustekinumab. In patients who have responded to treatment with ustekinumab, corticosteroids may be reduced or discontinued in accordance with standard of care

In Crohn's disease or Ulcerative Colitis, if therapy is interrupted, resumption of treatment with subcutaneous dosing every 8 weeks is safe and effective

Plaque psoriasis - moderate to severe
- After failure to respond to, or a contraindication to, or intolerance to other systemic therapies including ciclosporin, methotrexate (MTX) or PUVA (psoralen and ultraviolet A)
Psoriatic arthritis (Additional agent may be required)
- When the response to previous non-biological disease-modifying antirheumatic drug (DMARD) therapy has been inadequate

  • SC: Initial: 45mg at week 0 and 4; Maintenance: 45mg, every 12 weeks
    OR
  • SC: >100kg: Initial: 90mg at week 0 and 4; Maintenance: 90mg, every 12 weeks

In these patients, 45mg was also shown to be efficacious. However, 90mg resulted in greater efficacy

Consideration should be given to discontinuing treatment in patients who have shown no response up to 28 weeks of treatment

South African manufacturer recommendations

Plaque psoriasis - moderate to severe
- In candidates for phototherapy or systemic therapy
Psoriatic arthritis - moderate to severe (Additional agent may be required)

Mechanism of action

Ustekinumab is a fully human IgG1κ monoclonal antibody that binds with specificity to the shared p40 protein subunit of human cytokines interleukin (IL)-12 and IL-23. Ustekinumab inhibits the bioactivity of human IL-12 and IL-23 by preventing p40 from binding to the IL-12Rβ1 receptor protein expressed on the surface of immune cells. Ustekinumab cannot bind to IL-12 or IL-23 that is already bound to IL-12Rβ1 cell surface receptors. Thus, ustekinumab is not likely to contribute to complement- or antibody-mediated cytotoxicity of cells with IL-12 and/or IL-23 receptors. IL-12 and IL-23 are heterodimeric cytokines secreted by activated antigen presenting cells, such as macrophages and dendritic cells, and both cytokines participate in immune functions; IL-12 stimulates natural killer (NK) cells and drives the differentiation of CD4+ T cells toward the T helper 1 (Th1) phenotype, IL-23 induces the T helper 17 (Th17) pathway. However, abnormal regulation of IL 12 and IL 23 has been associated with immune mediated diseases, such as psoriasis, psoriatic arthritis, Crohn's disease and ulcerative colitis. By binding the shared p40 subunit of IL-12 and IL-23, ustekinumab may exert its clinical effects in psoriasis, psoriatic arthritis, Crohn's disease and ulcerative colitis through interruption of the Th1 and Th17 cytokine pathways, which are central to the pathology of these diseases.
