Special populations

Pregnancy

South African recommendations

  • Contraindicated
  • Safety in pregnancy has not been established
  • When pregnancy is detected, valsartan should be discontinued as soon as possible
  • Not to be used in pregnancy as teratogenicity has been shown in experimental animals
  • In case of accidental exposure to angiotensin II receptor antagonist therapy, appropriate foetal monitoring should be considered
  • Infants whose mothers have taken valsartan should be closely observed for hypotension
  • There have been reports of spontaneous abortion, oligohydramnios and newborn renal dysfunction when pregnant women have inadvertently taken valsartan
  • Valsartan acts directly on the renin-angiotensin-aldosterone system (RAAS) and therefore should not be used in women planning to become pregnant. Healthcare professionals prescribing valsartan should counsel women of childbearing potential about the potential risk during pregnancy
  • Women of childbearing age should ensure adequate contraception

Recommendations in other jurisdictions

  • First trimester: Caution; not recommended
  • Second and third trimester: Contraindicated
  • Angiotensin II receptor antagonists (AIIRAs) should not be initiated during pregnancy
  • Unless continued AIIRAs therapy is considered essential, patients planning pregnancy should be changed to alternative anti-hypertensive treatments which have an established safety profile for use in pregnancy
  • When pregnancy is diagnosed, treatment with AIIRAs should be stopped immediately, and, if appropriate, alternative therapy should be started
  • Epidemiological evidence regarding the risk of teratogenicity following exposure to ACE inhibitors during the first trimester of pregnancy has not been conclusive; however, a small increase in risk cannot be excluded. Whilst there is no controlled epidemiological data on the risk with AIIRAs, similar risks may exist for this class of agents
  • AIIRAs therapy exposure during the second and third trimesters is known to induce human foetotoxicity (decreased renal function, oligohydramnios, skull ossification retardation) and neonatal toxicity (renal failure, hypotension, hyperkalaemia). Should exposure to AIIRAs have occurred from the second trimester of pregnancy, ultrasound check of renal function and skull is recommended. Infants whose mothers have taken AIIRAs should be closely observed for hypotension

Breastfeeding

South African recommendations

  • Contraindicated
  • Safety in breastfeeding has not been established
  • It is not known whether valsartan is excreted in human milk
  • Since valsartan was excreted in the milk of lactating rats, mothers taking valsartan should not breastfeed their infants

Recommendations in other jurisdictions

  • Caution; not recommended
  • Because no information is available regarding the use of valsartan during breastfeeding, valsartan is not recommended and alternative treatments with better established safety profiles during breastfeeding are preferable, especially while breastfeeding a newborn or preterm infant

Fertility

South African recommendations

  • There is no information on the effects of valsartan on human fertility
  • Studies in rats did not show any effects of valsartan on fertility
  • Valsartan had no adverse effects on the reproductive performance of male or female rats at oral doses up to 200mg/kg/day, approximately 18 times the maximum recommended human dose on a mg/m² basis (calculations assume an oral dose of 320mg/day and a 60kg patient)

Recommendations in other jurisdictions

  • Valsartan had no adverse effects on the reproductive performance of male or female rats at oral doses up to 200mg/kg/day. This dose is 6 times the maximum recommended human dose on a mg/m2 basis (calculations assume an oral dose of 320mg/day and a 60kg patient)

Renal Impairment

  • No dose adjustment recommended
A somewhat higher systemic exposure to valsartan was observed in some elderly subjects than in young subjects; however, this has not been shown to have any clinical significance

Mechanism of action

Valsartan is an orally active, specific angiotensin II (Ang II) receptor antagonist. It acts selectively on the angiotensin 1 (AT1) receptor subtype, which is responsible for the known actions of angiotensin II. The increased plasma levels of Ang II following AT1 receptor blockade with valsartan may stimulate the unblocked AT2 receptor, which appears to counterbalance the effect of the AT1 receptor. Valsartan does not exhibit any partial agonist activity at the AT1 receptor and has much greater affinity (about 20 000 fold) for the AT1 receptor than for the AT2 receptor. The angiotensin 2 (AT2) receptor subtype is unrelated to cardiovascular effect.
