South Africa faces an imminent shortage of unfractionated heparin (UFH) injections, a critical medication for various medical conditions. The sole supplier, Fresenius Kabi, has been ordered by the South African Health Products Regulatory Authority (SAHPRA) to halt production, with existing supplies expected to last only four to five months. This manufacturing pause creates a significant risk of stock depletion, jeopardizing patient care.
In response, healthcare facilities are being urged to conserve their UFH stock for patients who have no suitable alternatives. Supplies should be prioritized for specific, high-risk groups, including pregnant women with certain heart conditions and individuals with severe kidney impairment or those on dialysis. For all other patients, clinicians should consider alternative anticoagulants, such as Direct Oral Anticoagulants (DOACs) like rivaroxaban, or Low Molecular Weight Heparins (LMWHs) like enoxaparin.
The guidance is clear: monitor your UFH supply closely and use it only when absolutely necessary. This strategic rationing is crucial to ensure that the most vulnerable patients continue to receive the treatment they need until the supply chain is restored. Clinicians should familiarize themselves with the updated treatment guidelines to effectively manage the transition to alternative therapies.
