Infection Fungal skin infections
Brief outline/clinical setting May cause primary infection of the skin or present with cutaneous manifestations of systemic disease
Aetiologies/organisms Candida spp, Dermatophytes (Trichophyton, Epidermophyton, Microsporum), Tinea versicolor (Malassezia furfur)
Others: Aspergillus spp, Mucormycosis (Mucor, Rhizopus), Pseudoallescheria boydii, Fusarium spp, Sporothrix schenkii, Emergomyces
Optimal antimicrobial selection
  • Candida spp:
    • Vulvovaginitis: single application of topical anti-fungal agent (e.g. clotrimazole) OR fluconazole 150mg po single dose
    • Balanitis: topical antifungal agent for 1-3 weeks
    • Chronic mucocutaneous candidiasis: fluconazole 100-200mg po for 7-14 days
    • Mastitis: if topical azole (e.g. clotrimazole) therapy fails, fluconazole 200mg po for 14 days is advised
    • Diaper dermatitis with Candida superinfection: topical antifungal applied 2-3 times per day until rash has resolved
  • Dermatophytes:
    • Tinea pedis / corporis / cruris: topical antifungal (e.g. azoles, terbinafine) applied 1-2 times per day for 1-3 weeks
    • Tinea capitis: systemic antifungal e.g. fluconazole 200mg po weekly for 6 weeks (children: 6mg/kg daily for 28 days)
  • Tinea versicolor/Pityriasis versicolor: selenium sulphide 2.5% suspension. Lather shampoo on affected parts and leave on for 30 minutes. Repeat x 3 consecutive days
Others: Consult infectious diseases for optimal treatment selection, based on clinical presentation and culture results
Level of evidence Guidelines (Strong recommendation, high quality evidence: Candida)
RCT, systematic review (dermatophytes, tinea versicolor)
