Reporting Systems

District level hospital guidelines applicable for interns and comm service doctors. Published in 2016

The Department expects that staff of all levels including students report in matters that in one way or another affect healthcare provision or its quality to the children and families we serve.

Healthcare delivery may be affected by a variety of causes and unless these are reported in timeously the problems experienced are less likely to be addressed in good time.

It is a healthcare rights advocacy issue as well as a professional and ethical duty to report issues that affect patient management and ultimately outcomes.

To maintain silence on a witnessed adverse outcome or compromised care resulting from a possibly avoidable factor or system failure could be regarded as a dereliction of duty and in some cases may be taken up with the Health Professions Council of South Africa.

The Security Incidents Reporting Form relates to the registering of any security related issue experienced or witnessed by a staff member. It is essential for the hospital authorities to ensure that these reports are submitted so as to improve the overall security of the institution as well as deliver an excellent security service.
