Mechanism of action

Lamivudine

Lamivudine is a nucleoside analogue which has activity against human immunodeficiency virus (HIV) and hepatitis B virus (HBV). It is metabolised intracellularly to the active moiety, lamivudine 5'-triphosphate. Its main mode of action is as a chain terminator of viral reverse transcription. The triphosphate has selective inhibitory activity against HIV-1 and HIV-2 replication and is also active against zidovudine-resistant clinical isolates of HIV.

Stavudine
Stavudine is a synthetic thymidine nucleoside analogue. It enters cells by diffusion and is phosphorylated by cellular kinases to the active metabolite stavudine triphosphate. It inhibits HIV reverse transcriptase by competing with the natural substrate, thymidine triphosphate. It also inhibits viral DNA synthesis by causing DNA chain termination due to a lack of the 3'-hydroxyl group necessary for DNA elongation. It further inhibits cellular DNA polymerase and gamma, and markedly reduces the synthesis of mitochondrial DNA.

Nevirapine
Nevirapine is a non-competitive, non-nucleoside reverse transcriptase inhibitor of HIV-1. It binds directly to reverse transcriptase (RT) and blocks the RNA-dependent and DNA-dependent DNA polymerase activities.
